i. Purpose

This grant application invites applicants to receive grants from the Older Americans Act (OAA), Aging Services Grant for Wyoming. The purpose of the grant funds are to provide Title III-D Disease Prevention and Health Promotion Services to older adults living in Wyoming communities, outside of institutional placement.

Awardees will receive funding for approved projects not to exceed 24  months of service.

ii. Background

This Title III-D application regards the $327,226 (WYOAPH) for Disease Prevention and Health Promotion Services under Title III-D of the OAA appropriation to the Wyoming Department of Health, (WDH) Aging Division (AGD).

iii. Objective and Scope

The objective of this application is to fund projects that demonstrate meeting community needs of older adults in Wyoming. The Disease Prevention and Health Promotion, Evidence-Based Program, is both educational and a support program, designed to assist older individuals in the self-management of their physical and mental health. This program is designed to help in the prevention of, and progression of chronic disease. This program promotes healthy eating, and regular physical and mental activities, which are the keys in helping to promote behaviors that can help older individuals live a longer and healthier life. The goals of the program are to provide programs to assist elderly Americans by removing barriers to independent living and to educate and implement preventive health services and behavior modification for patients to take an active role in managing their chronic disease. Implementing Evidence-based programs that are proven effective to provide the education, knowledge, and skills necessary to improve the quality of life that may prevent/lessen the severity of complications resulting from chronic disease.

Title III-D funds will have to meet the criteria equivalent to the "highest-level" criteria of the Health and Human Services (HHS) certified evidenced-based program definition. Disease Prevention and Health Promotion, Evidence-Based Program (EBP) may only be used for programs and activities which have been demonstrated through rigorous evaluation to be evidence-based and effective. Projects under this application are limited by federal law. Each application will need to expressly establish how the proposed project is permitted to be funded under the Older Americans Act of 1965, as amended through P.L. 116-131, enacted March 25, 2020.

iv. Reporting Requirements

Grants awarded under this application are subject to the following reporting requirements:

For grant awardees providing direct services to eligible participants, the awardee must collect documentation as prescribed by WDH - AGD and report no less than annually the types of services provided in addition to participation rates for each eligible participant. Depending on the scope of the project, the WDH - AGD will coordinate with successful applicants to determine the most appropriate form of reporting to satisfy federal grant monitoring requirements. 

v. Funding Match Requirements

Grants awarded under this application do not require a State and Local match requirement.
